Fifty vials of Moderna vaccine were left outside the pharmacy fridge overnight over the weekend in Aurora Medical Center-Grafton.
Ongoing coverage: Coronavirus in Wisconsin
Lawyer Aurora Health said in a statement that the moderna vaccine vial “was inadvertently removed from a pharmacy refrigerator overnight.”
Each vial contains 10 doses.
Advocate Aurora Health said some of the vaccine was given to team members on December 26 within the approved 12-hour cooling window, but most of the vaccine had to be discarded due to temperature requirements.
An internal review determined that the vials were not replaced after being temporarily removed to gain access to other items.
